For Prescription Refills

Please include the patient’s full name, date of birth, primary care physician, the name and strength of the medication and pharmacy information. Please note whether a 30 or a 90-day supply is needed and allow two business days for processing requests.
For Insurance Referrals

Please include the patient’s full name, date of birth, primary care physician, reason for the referral, name of the physician to whom the referral is needed and the date of the appointment if applicable. Please allow one week for processing requests.

Please include the patient’s full name, date of birth, date the test was completed and the type of test (i.e. lab, X-ray, biopsy, stress test etc.) and where the test was performed. Please allow two business days for a response and indicate the preferred method of contact (telephone, email, etc.).
